About Donation
In Ohio, you can list your desire to be a donor when you apply for or renew your driver’s license. You can also declare your intent to donate by signing up through the online registry at Donate Life Ohio, or print and complete a donor registration form and return it to the Bureau of Motor Vehicles.
Ohio law says that registered donors have consented to donation and that permission from the next-of-kin is not required at the time of the donor’s death. Completing the registration process creates a legal record of your consent to donate.
You can choose to donate only specific organs and tissue or any body parts that are needed. You may indicate how your organs and tissue may be used: for any purpose or for transplant, research, or medical education only. You may also complete this form to specify that you do not want to be a donor.
It is still important to discuss your wishes with your family. If you have not registered, your family will be asked to consent to donation. Even if you have registered as a donor, your family will find comfort in knowing that your wishes to be a donor are being carried out.
